Instructions for filling out your fishing license application

1. In the top left box you should write in your passport number.

2. Social Security Number: Leave this blank

3. Print your name clearly. Use Roman letters, not kanji.

4. Your height and weight may not be written using the metric system. Your height should be in feet and inches. If you are not sure how to convert your height and weight from metric, you can guess. An average Japanese male, age 18 to 22 is between 5 feet 6 inches and 6 feet 0 inches, and his weight is between 130 pounds and 180 pounds. An average Japanese female, age 18 to 22 is between 5 feet 2 inches and 5 feet 8 inches, and her weight is between 95 pounds and 150 pounds. If you need help, ask your professor.

5. Eye color: Most Japanese have brown eyes.

6. Date of birth: Write it out completely, like this: September 12, 1989.

7. Minnesota driver’s license: On this line you should write the word “Passport” followed by your passport number.

8. Address: Write out your full address using Roman letters, not kanji.

9. Area code- Telephone number: Write out your phone number in Japan, or the phone number of your parents.

10. Applicant’s signature and date: Sign your name here using Roman letters, not kanji. Write today’s date after your name.

11. Mail to: Do not send your application to the address shown on the application. Mail the application and a copy of your passport (the inside, showing your picture and address) by to this address:
